Hipokrat gibi Galen de bütün hastalıkları birtakım sıvıların fazlalığı temelinde sınıflandırdı sınıflandırdı. Kızıl renkli, sıcak ve ağrılı bir şişmeyi betimleyen enflamasyon kanın; hepsi de soğuk, çamursu ve beyaz olan tüberkül, püstül, katar ve lenf düğümcüğü balgamın; sarılık da sarı safranın fazlalığından kaynaklanıyordu. Galen, sıvılardan en kötücül olanını ise kansere saklamıştı: Siyah Safra. Bu yağlı, ağdalı sıvıya atfedilen yalnızca bir hastalık daha çıkacaktı: tepeden tırnağa metaforla kaynayan depresyon. Depresyon için ortaçağda kullanılan ‘melankoli’ sözcüğü, adını Yunanca melas(siyah) ve khole(safra)’den alıyordu.
